Engine fails over Ottawa after crankcase sealant was misapplied in maintenance

In July 2025, a privately registered AA-5A's engine failed over Ottawa and the aircraft hit trees about 960 m short of the runway; the pilot died and 2 passengers had minor injuries. The TSB found LOCTITE-515 sealant misapplied during earlier engine maintenance, on crankcase mating surfaces, through bolt threads, and main journal bearings.

SureSight's step-by-step verification records each reassembly step on camera, such as sealant applied only where the service instruction allows, so an off-spec step can be flagged before sign-off.

Steel beam falls into molten zinc after lifting chains missed their mandatory inspection

In October 2021, a lifting chain failed at Wedge Group Galvanizing's site in Crediton, Devon, and a steel beam of nearly three tonnes fell into a bath of molten zinc; a worker suffered 40 percent burns. HSE found the chains had signs of damage and deterioration and had not had their mandatory regular inspection. The company was fined £566,666.

SureSight records each lifting-gear inspection with point-of-work imagery and flags visible chain damage or a missed check, so worn slings can be withdrawn before use.

Ship crane motor falls on a crew member after a seen defect went unlogged

In June 2025, an engine room crane motor on the bulk carrier RTM Weipa fell about 6 meters and struck a trainee, causing minor injuries and minor equipment damage. The ATSB found the crew had seen the motor wobbling about four months earlier, but it was never inspected or logged in the ship's maintenance system.

With SureSight, a defect noticed during work is photographed and logged against the equipment on the spot, and stays open until someone inspects and closes it.

Fire door frames missing on lifts at 2 public estates and 338 private buildings

In October 2026, surprise inspections by Hong Kong's EMSD found fire-resisting doors on 48 public-housing lifts fitted without frames, though the work was reported complete. EMSD then found frame components missing at 338 private buildings, all with lifts from the same contractor. Police are investigating.

SureSight records each installed component with time-stamped imagery, so a missing part shows as an unconfirmed step before sign-off.

MEMU train derails after a rail panel left too close to the track strikes a wheel

In October 2026, a MEMU train derailed near Kherli station in Rajasthan, India, at a rail renewal worksite; no one was hurt, and several trains were diverted. A preliminary inquiry found a rail panel kept at an unsafe distance from the track struck a wheel, and basic measures to separate the panels from the running track were missing.

SureSight can make worksite set-up a recorded step: stored rail beside the running line is confirmed on camera before trains pass, and anything too close is flagged.

Light aircraft engine stops with a fuel filter screen missing and debris in the fuel

In April 2025, a Zenair light aircraft's engine stopped near Tatenhill Airfield, Staffordshire; it landed heavily and was damaged beyond economical repair, though neither occupant was hurt. The AAIB found debris in the fuel system, the gascolator's filter screen and gasket missing, and pre-flight fuel drain checks not carried out.

SureSight's verified reassembly records each part as it is fitted, so a missing filter screen or gasket shows as an unconfirmed step, not a completed one.

Cruise ship lift moves while a technician is in the shaft during an inspection

In October 2025, a passenger lift on the cruise ship Arvia moved while a technician was inside its shaft during an inspection, and he was fatally injured. The MAIB found control of the lift had not been established first: inspection mode and the emergency stop were not set, and the work relied on self-resetting door interlocks.

SureSight can confirm on camera, and record, that required controls such as inspection mode and the emergency stop are set before anyone enters a shaft.

After a deadly plant explosion, inspectors find missing safety valves and flame arrester

In June 2026, an explosion in a cleaning facility at Hanwha Aerospace's plant in Daejeon, South Korea, killed 5 people and injured 2. A labor ministry special inspection then found 473 violations, including missing safety valves, a diesel tank with no flame arrester, and equipment not certified as explosion-proof in explosion-risk areas.

SureSight's independent inspection records each safety device, such as relief valves, flame arresters, and grounding, with imagery, so a missing one is flagged and tracked until it is fixed.

Former MBTA track inspector pleads guilty to falsifying Red Line track inspection reports

From January to October 2024, MBTA Red Line track inspection reports were falsified instead of the inspections being done, federal prosecutors said. In September 2026, a former track inspector pleaded guilty to conspiracy to falsify records and related charges. Prosecutors said reports used train numbers found online and had durations extended.

SureSight's point-of-work verification ties each inspection step to recorded imagery taken on site, so a report with no matching evidence stands out in audit-ready records.

Crude oil leaks from a pipeline repair sleeve whose repair records could not be found

In September 2026, crude oil seeped from the ground into a storm drain in Sterling Heights, Michigan, from a failed 2010 repair sleeve on an Energy Transfer pipeline; at least 15 barrels were recovered. PHMSA said the company could not locate the sleeve's repair records, or records to verify 11 other sleeves on the line. The cause is still unknown.

SureSight keeps an audit-ready, image-backed record of each repair step, such as sleeve fit-up and welding, tied to the asset, so the work can still be verified years later.

Freight train derails at a switch adjusted twice without testing or records

In July 2025, a Norfolk Southern freight train derailed 24 cars at a spring switch near Browns, Illinois, breaching three hazardous-materials tank cars, injuring two crew members, and causing $9.2 million in damage. The NTSB found the switch had been adjusted twice beforehand without the work being recorded or the required tests run.

SureSight logs each repair with imagery tied to the asset, so the work is on record even without a form. A repair that needs a test stays open until the test is confirmed.

Police say unmanaged sludge and poorly inspected exhaust systems let a plant fire spread

In March 2026, a fire at Anjeon Industry's car parts plant in Daejeon, South Korea, killed 14 employees and injured 60. Police believe unmanaged sludge and poorly inspected and maintained local exhaust systems let it spread rapidly; fire doors were also kept open. Police and labor authorities said the plant had 48 fires in five years.

SureSight records each scheduled exhaust-system check and housekeeping step with imagery, so sludge buildup or a fire door left open is flagged, and the finding stays open until it is fixed.

747 freighter declares MAYDAY after rainwater reaches electrical units

In February 2024, rain entered a Boeing 747 freighter through a door left open during overnight maintenance and knocked out two electrical buses after takeoff from Heathrow. The crew declared MAYDAY and diverted to Amsterdam; no one was hurt. The AAIB found sealant missing at the door and no required re-inspection of the drip shield.

SureSight makes visual inspections recorded and repeatable: each seal and shield is viewed on camera and logged, so missing sealant can be flagged with a photo.

Commuter train derails where tie plates held fewer lag screws than required

In February 2026, a GO Transit train derailed leaving Toronto's Union Station, damaging track and signals and delaying service up to about 90 minutes; no one was hurt. Metrolinx found tie plates with fewer lag screws than its 2025 standard required, plus broken, loose, or missing screws. A corridor check found 40 more substandard locations.

SureSight checks each fastening point on camera against the required pattern. A missing or broken fastener is flagged with imagery and stays open until the repair is confirmed.

Tanker engine room fire: fuel line not isolated, though the checklist was marked done

In May 2025, diesel ignited in the engine room of the tanker Scot Stuttgart in San Juan after the crew removed fuel tubing from a generator without isolating the fuel return line. Damage was estimated at $333,525; no one was hurt. The NTSB noted both isolation items on the work permit had been marked complete.

SureSight marks an isolation step done only when the camera confirms it at the point of work. A step it cannot confirm is flagged before the line is opened.

Trolley pantograph snags sagging overhead wires; NTSB cites no formal inspection program

In September 2025, a SEPTA trolley's pantograph snagged overhead wires in Springfield, Pennsylvania, and two of its arms pierced the passenger compartment. Four people sought hospital care for minor injuries, damage was about $254,000, and the line closed for 18 days. The NTSB found SEPTA had no formal inspection program for the wires.

SureSight logs each scheduled inspection with time-stamped imagery, so a missed one shows as a gap. Defects found are photographed and stay open until a repair is confirmed.

Freight train derails where a track buckle was straightened but the rail not destressed

In July 2025, a freight train derailed 21 railcars near Sardis, Mississippi, releasing about 10,500 gallons of sodium hydroxide; damage was about $2 million. The NTSB found a track buckle had been straightened 5 days earlier without destressing the rail, and required hot-weather inspections were not performed on 26 of 38 days.

SureSight records each repair step with imagery, so a step like destressing the rail stays open until confirmed. Every scheduled inspection is logged, so a missed one shows as a gap.
